This is where you will fill out a cosmetic tattoo request.

We will then set up a consultation. After the consultation you will put down a deposit ($100, that will cover the last hour of your procedure) and get booked on the calendar. All deposits are non-refundable. We require a 7-day notice for rescheduling. After the second reschedule you will lose your deposit.
